What's involved

Initial evaluation
Comprehensive face-to-face (in person or video) assessment per 49 CFR Part 40.281. Clinical interview, biopsychosocial history, substance-use history.

Recommendation
Written recommendation for education, treatment, or both. Specific provider list when applicable.

Treatment compliance monitoring
Coordination with the recommended education or treatment provider for adherence verification.

Follow-up evaluation
Final face-to-face evaluation per § 40.305. Assesses successful compliance and determines return-to-duty eligibility.

Return-to-duty report
Signed SAP report sent directly to the Designated Employer Representative (DER) per Part 40 requirements.

Follow-up testing plan
Written follow-up testing plan with minimum requirements per § 40.307. Plan stays with the employee through re-employment.
